Case Description
I am 56 years old with excellent physical and dental health. No serious dental procedures have been required untill two years ago when a crown and subsequent root canal was completed on #14 due to constant sensitivity and lingering pain. The pain was not unbearable, just always there. The tooth was extracted a year ago, but the area still tender and now #15 and #16 are beginning to bother me. I have recevied many xray scans, and examinations by several dentists and dental surgeons. No one seems to know what is causing the pain/discomfort.

I am now considering a very expensive implant due to the loss of bone from the extraction Estimated fee is $5,459 which includes: bone replacement graft, osseous/other graft, platlet rich plasma, surgical placement of implant #14; and regular sedation of area. This, of course, does not include the cost of the crown or heaven forbid gum surgery.

Would appreciate your advise on:

1. Should I proceed with the impant and might this improve the unknown sensitivity and slight pain in the area. 2. Are my cost estimates reasonable and what concerns should I have on the procedure such as the material used for the bone graft and other things that can go wrong, but you never hear about until it is too late.

You should determine what needs to be done to alleviate the pain/discomfort you currently have before even thinking about implant placement. There are simple diagnostic tests that can be done and together with the available x-rays should properly diagnose the problem. There are no "hospital scans" better than the dental x-rays you probably have; given the opportunity they might suggest MRIs or CT scans. Do that if you wish but to me it seems unnecessary and probably unhelpful. A careful history (medical and dental) needs to be done first then a careful study of what and why was already done to the affected area. An endodontist should be your first choice.
